A medical reflex hammer and a tuning fork are two of the oldest tools in a clinician's bag, but they are not relics. They are precision instruments for detecting and quantifying abnormalities in the human nervous system. The reflex hammer is used to elicit a deep tendon reflex, such as the knee jerk or the bicep reflex. The tuning fork is used to assess sensory pathways, specifically the perception of vibration and, to a lesser extent, hearing. Together, they provide a window into the functional integrity of the peripheral nerves, the spinal cord, and the brain itself. How a Reflex Hammer Works

The clinical term for the response a reflex hammer elicits is the muscle stretch reflex, also known as the deep tendon reflex. Despite the name, the reflex is not initiated by striking the tendon itself, but by stretching the muscle attached to that tendon. When the patellar tendon is struck, it tugs on the quadriceps muscle, briefly stretching it. This mechanical stretch is detected by specialized sensory receptors called muscle spindles. The spindle fires a signal along a sensory nerve to the spinal cord. There, the signal synapses directly onto a motor neuron, which sends an impulse back to the same muscle, causing it to contract.
